A randomized controlled study of 134 medical residents compared communication training outcomes using real-time voice generative AI-based virtual patients (RT-VP), standardized patients (SP), and peer role-play (PR). The RT-VP was developed on the Doubao platform and demonstrated SPIKES communication performance scores (24.9 ± 2.9) comparable to SP (26.6 ± 3.0), significantly outperforming PR (20.4 ± 4.3). This research addresses the global challenge of scalable, authentic communication training in clinical education.
