Description
39 publications describing 32 primary health care interventions for diabetes, hypertension, and dyslipidemia in Malaysia, mapped against the Chronic Care Model. The dataset was compiled by Xin Rou Teh via a scoping review of PubMed, Embase, Scopus, and MyMedR up to December 2024.
