Home Medication Review Checklist Development and Validation Study

A 65-item Home Medication Review (HMR) checklist was developed and validated for standardizing pharmacist-led medication reviews. The checklist covers 12 domains and demonstrated high content validity with an average Cronbach’s alpha of 0.97. It was tested in a feasibility study involving 20 participants, showing 93.5% pharmacist compliance.
