A 58-year-old woman with long-standing relapsing–remitting MS developed acute severe myelitis, a case documented by figshare admin karger in May 2026. Serum testing revealed high-titre MOG antibodies, and spinal MRI showed transient central grey matter involvement with an H-sign. This report highlights diagnostic challenges when distinguishing MOG antibody–associated disease from MS relapse.
