Chronic Heart Failure Discharge Preparedness: Patient and Clinician Perspectives

This qualitative study contains interview data from 15 chronic heart failure patients and 6 cardiovascular clinicians at a tertiary hospital in urban China. The data was collected from November 2024 to March 2025 and analyzed using directed content analysis to identify four themes of misalignment in discharge planning.
