Marguerite Burns at the University of Wisconsin–Madison compiled this dataset using federal waiver documents, state reports, policy organization analyses, and news articles. Onsite data collection at the Centers for Medicare and Medicaid Services in January 2016 was conducted to review non-digitized earlier documents.
